Me, myself and IAnother Exemption Obtained for 40 year oldSat Oct 10, 2009 13:32126.96.36.199With the economy being as bad as it is. I decided to go to Grad school and pick up another degree. So I apply and get accepted to SUNY Maritime College I am 40 years old. (Ft Schuyler, Bronx, NY). As I am filling out the paperwork, I see yet another very elaborate proof of immunization form, to be completed by my doctor.
As I have no family physician, I receive medical care ala carte from a Physicians Assistant. I am fully vaccinated (preseumably) but I have no idea where any of the documents are after many recent moves.
So I fill out the NYS Education Department exemption form and hand it in, mixed with the rest of the paperwork.
I'm assuming my request was approved because I have been in school since August with not so much as a whisper from the Health Services people.
So even if you do not have school age children, you can get some mileage out of the exemption forms. BEats the hell out of getting all those shots again.
