You are being silly.

The ONLY time that a question can POSSIBLY even come up is if an individual is trying to use a bathroom that doesn't match their physical appearance.

Unless the second person goes into a men's room and loudly announces to all and sundry, "Yes, I know I look like a tough biker dude, but I was born with a vagina." there will be no issue.
