Hi John:
There was No lock washer installed. I'm guessing that is the problem. Can you recommend an appropriate washer to use in terms of grade, material, etc. Thanks in advance.

          Hello John, I suspect that the split helical lock washer for hex socket head cap screws is all that will fit properly. JOhn Stimson
